I am the parent or legal guardian of the young people listed and I consent to their enrollment. I consent to the young people listed taking part in YES America programs and activities, and I understand that, like any activity, they carry some risk of injury. In an emergency, if I can’t be reached, I authorise YES America staff to arrange emergency medical treatment for the young people listed.
